We offer both individual and group therapy.

Individual therapy begins with assessment.  Our assessment can take place in the home or the clinic, and includes testing and observation with your child, as well as an interview with the parent or guardian. A report is provided with therapy recommendations, which we review with you in a follow up session.

Groups will be running beginning in 2018. Please stay tuned for more information.

Referrals are made by contacting our office in one of three ways:

  1. Modern OT Online Referral Form
  2. Telephone 613-792-3461
  3. Email referral@modernot.ca

After we receive the referral, we ask that you fill in an Intake Form, which provides the OT with some some preliminary information about the child – milestones, observations, concerns.  The OT will review the form and use this information to prepare appropriate activities to optimize your child’s assessment process.

A copy of the PDF Pediatric Intake Form is below.  This form can be downloaded. We can also send this form to you.  Please return it to our office prior to the assessment, via email referral@modernot.ca or fax 613-792-3462.
