Hospital Discharge Coordination
Often, the first contact with one of our occupational therapists will take place before an individual goes home from the hospital after an event such as a car accident. We will come to the hospital to meet with the individual and the hospital treatment team in order to coordinate a safe and timely discharge from the hospital. The occupational therapist will also complete an assessment in the home prior to discharge to address, recommend and coordinate needs for assistive devices, equipment, environmental modifications as well as necessary personal care assistance to facilitate the discharge process.
A follow-up visit takes place in the home to ensure that all needs are being met, and that equipment is set up correctly and safely for the duration of recovery.
