Occupational Therapy

Our occupational therapists can help with decision making for future living arrangements. We can assess the needs of individuals wishing to remain in their home, and help to make recommendations regarding assistance, services, and assistive devices when considering independent vs. supported living environments. Assessments of needs can also take place in residential and long-term care facilities, in order to ensure that the needs of the individual continue to be met for optimal function and quality of life.

Our occupational therapy approach is to be comprehensive in our assessment, educational in our sessions, and practical in our recommendations. We seek cost effective, realistic solutions in order to maintain and enhance quality of life in the aging process.

Community Care Coordinator / Case Management

Seniors and their families sometimes feel uncertain about navigating our current health care system and community programs in order to implement plans that can address day-to-day needs.

For this reason, working with a Community Care Coordinator (CCC) helps to ease uncertainties by providing a primary point of contact for seniors and their family members to manage treatment services, programs, and care options. In consultation with the senior and their family, this can include initial guidance in developing an overall plan of care within the community, to direct support and advocacy for the patient in the event of an emergency or crisis.
