Mobility Assessments ADP

Wheelchairs – Walkers – Scooters

Our occupational therapists are able to assess your needs for mobility and adaptive equipment, including manual and power wheelchairs, walkers, and scooters.

Assistive Devices Program

The Assistive Devices Program (ADP) is a program offered through the Ministry of Health and Long-Term Care to provide support and funding to Ontario residents who have long-term physical disabilities and to provide access to personalized assistive devices appropriate to their individual needs.

We have occupational therapists on our team who are able to provide assessments and complete applications to the assistive devices program for wheelchairs, positioning, and ambulatory aids.

General Funding Eligibility

In considering eligibility for an ADP mobility device, the authorizer must consider the following as outlined by ADP:

  • The device that meets the applicant’s basic mobility requirement;
  • The device must be used on an ongoing daily basis; and
  • The required and intended use of the device must be to provide mobility on an ongoing daily basis within the applicant’s place of residence and to gain entry/exit from the applicant’s place of residence.

Scooter ADP Funding Eligibility

In addition to the General Funding Eligibility, those applicants wishing to be assessed for a power scooter must also meet the following criteria:

  • Power mobility is basic as defined by the ADP for funding purposes;
  • The applicant does not have seating requirements now or in the foreseeable future;
  • The scooter is not intended to be a replacement for a car or another transportation modality; and
  • The applicant can transfer independently on to and off the scooter.

Mobility assessments take place with the occupational therapist in your living environment. The OT will also coordinate a consultation with a vendor to trial appropriate equipment. Following the assessment, the occupational therapist completes the required application forms to the ADP, who will then notify you directly regarding approval. The ADP does not fund for the assessment process.

Additional information regarding the Assistive Devices Program can also be obtained from the Ministry of Health and Long Term Care.

Assessments take place with an ADP-certified occupational therapist at our clinic or in your home (additional costs for in-home).
The assessment is completed within a few hours, however, the ADP process is about 3-6 months in total.
To qualify for the ADP, you must:
  • be an Ontario resident.
  • have a valid Ontario health card.
  • have a disability requiring the equipment or supplies for six months or longer.
You do not qualify for the ADP if you:
  • are already receiving financial support for the same equipment from the Workplace Safety and Insurance Board or Veterans Affairs Canada.
ADP covers the cost of 75% of the mobility device, but they also DO NOT cover the cost of this assessment. You will have to pay out of pocket or use a third-party funder for the assessment and the additional 25% of the device.

We would require direct approval from ODSP. We can send a fax request for funding of this assessment. We would require your ODSP case number, case manager’s name and fax number, and your basic contact information.
