About Us

Modern OT was originally founded in 1998, and is operated by two occupational therapists, Fiona Smith Bradley and Shannon McGrath.   In 2022, Modern OT joined Lifemark Health Group to grow occupational therapy across their organization. Together, Fiona and Shannon have built a team with a common goal of bettering the lives of others with an innovative, client-centered approach in occupational therapy.

Modern OT is Eastern Ontario’s largest occupational therapy group. With over 40 occupational therapists spanning three clinic sites – Ottawa, Kingston and Pembroke – we are able to service individuals along the 401 corridor from Belleville to Cornwall, east of Ottawa in places like Hawkesbury and Limoges, in Lanark County, Perth, and through the Ottawa Valley to Deep River and Barry’s Bay.

We conduct assessments and provide treatment with an experienced, evidence-based rehabilitation approach.  We have over 20 years of experience working with individuals recovering after motor vehicle accidents, workplace injuries, and returning to work after short or long-term disabilities.

Our services are not funded by OHIP, but many insurers cover occupational therapy.  Please contact your insurance provider to inquire about your coverage.

Modern OT offers private assessments in your home at two levels and cost structures:

In-Home Targeted Assessments are provided by an occupational therapist in order to identify urgent safety and accessibility issues within your home.

This assessment is for individuals who have an acute / urgent need for OT, for example following hospitalization, joint replacement, change in health status, or incidence of fall(s) at home.

  • The targeted assessment includes functional testing (i.e., mobility, cognition, balance) to focus on the specific (targeted) problem areas identified at the time of referral.
  • Within this assessment, it is reasonable to expect the OT to have time to address 1-2 areas of the home, such as the bathroom and/or the bedroom and/or the kitchen.
  • If more than two areas require assessment, then a Global Assessment is recommended.

This assessment targets urgent safety needs with recommendations for equipment, services, support that are needed as soon as possible to address the concerns of the individual.

The assessment lasts approximately 2 hours.

  • Yes. If health conditions or functional problems are present that can be addressed in therapy, occupational therapy treatment recommendations will also be provided. Fees for occupational therapy treatment are outlined below.
  • The OT will schedule a telephone call with the client 1-2 weeks after the assessment at no additional charge to follow up regarding recommendations made if no treatment session has been booked.

Yes. Once the immediate safety recommendations are in place, either before or after the client has had some OT treatment, we can then do a more global assessment to make recommendations for a longer-term living plan for the client.

  • The OT will provide a written summary of the recommendations to the client at the end of the assessment.
  • If a more formal report is required to be sent to a medical professional, insurance company, or other, then a report can be requested, and will be prepared by the OT at an additional cost.

Yes. The OT would be able to schedule a 30-minute telephone call with caregivers to discuss the assessment findings. The fee for the caregiver call is $65.

No. After a Targeted Assessment, the OT hourly treatment cost structure would apply for all follow-up, including to complete the remaining components of a Global Assessment.

Global Assessments are provided by an occupational therapist in order to identify long-term safety and accessibility issues within your home.

This assessment is ideal for individuals who recognize that their needs are changing due to factors, including health and aging, and would like to proactively assess their functioning in their home environment for recommendations to address present and future needs. ion Sample Description

This assessment involves the functional assessment of the client and observation of the multiple areas of the home to determine areas where short-term and long-term solutions may be addressed.

  • Following the assessment, the OT will provide recommendations for therapy, assistive devices, assistance, and/or environmental modifications to increase an individual’s safety in their home.
  • Short-term and long-term recommendations are provided.
  • Please note that additional recommendations may arise if over the course of occupational therapy with changes to a client’s health status or living arrangements.
  • Yes. The first step would be to have a Global Assessment.
  • When larger accessibility renovation projects are required, the occupational therapist assists by collaborating with accessibility design specialists and/or contractors throughout the planning stages, ensuring that the physical needs of the individual are met in the design.
The Global Assessment lasts approximately 3 - 3.5 hours.

If health conditions or functional problems that can be addressed in therapy are present, follow-up treatment will be recommended.

  • The OT will provide a written summary of the recommendations to the client at the end of the assessment.
